Caregiver Award Nomination
At Chancellor Funeral Homes, our families and staff hold a deep respect and appreciation for caregivers. This includes physicians, physician assistants, nurse practitioners, nurses, chaplains, social workers, CNAs, administrative professionals, volunteers, and all others who faithfully serve in this vital role.
We recognize the many emotional challenges that come with these positions. The hours are often unpredictable, and the circumstances can be difficult. To honor the dedication of those in this demanding profession, we have created the “Heart of Our Community Award”. This award celebrates individuals who go above and beyond in serving patients and their families, demonstrating patience, compassion, and kindness.

This award celebrates individuals who go above and beyond in serving patients and their families, demonstrating patience, compassion, and kindness. Each month, we will recognize one caregiver who has been nominated by peers, friends, patients, families, or anyone who wishes to highlight the impact of their work.
The monthly recipient will receive an award and a gift card. At the end of the year, we will host a banquet to honor all monthly recipients. During this celebration, a Review Committee will select one individual as “Caregiver of the Year,” who will receive a special gift.
